We will obtain your records and review past actions taken by Social Security concerning your case. We can help you complete the necessary forms and meet the filing deadlines required by the Social security Administration in order to assure continued consideration of your claim.

And if all efforts with Social Security fail, we will represent you at a hearing before an administrative law judge.

You will need to provide all of your Social Security papers including:

• Denial notices
• Decisions
• Appeal forms
• Letters and other correspondence to and from the Social Security Administration 

To review your case we will also need your doctor’s names, addresses and the number of times you have received treatment. We will need the names and addresses of all the hospitals where you have been treated as an inpatient or an outpatient.

We will need a list of each medication you are presently taking, the dosage and the names of the prescribing doctors.

You must provide the names and addresses of your employers during the last 15 years, the dates of employment by each employer and a brief description of your job duties to each job.

If we accept your case, John J. Ingram and Associates will take it on a contingent fee basis of a percentage of past due benefits in accordance with Federal regulations. Social Security regulations require that all fees be approved by the Social Security Administration.

There is no consultation charge and no fee unless your claim is successful. However, you will be obligated to pay any out of pocket expenses incurred such as charges for copying medical records and payments to doctors for medical reports.

Claimant agrees that if SSA favorably decides my claim at any stage through the first hearing at the Administrative Law Judge (ALJ) level of appeal, I will pay a fee of either (a) twenty-five percent (25%), or (b) $6,000. or the applicable maximum amount set by the Commissioner pursuant to section 206(a) (2) (A) and 1631(d)(2)(A) of the Social Security Act of all past Due benefits resulting from my claim, whichever is less.
